FIRST VISIT

First Visit


Your initial appointment will consist of a complete exam and full series of x-rays to help assist Dr. Holehouse in the proper diagnosis of your dental health. After this exam has been completed we will explain your diagnosis in detail and any treatment options if it applies, answering any questions and concerns you may have.

Please assist us by providing the following information at the time of your first appointment:

A list of medications you are presently taking. If you have dental insurance, please present you insurance card. This will save time and allow us to help you process any claims.

Please feel free to print up the office forms listed on Patient Forms page, complete, and bring with you at your appointment. You may also make a request for us to mail these forms to you when you make your appointment. To print up these forms please see the tab : Patient Forms. X-Rays: If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office, however we recommend for you to have them sent to you directly. This way if the records have not reached our office by the time of your visit, there will not be an out of pocket expense to you. If additional films are necessary, they can be taken at our facility.

IMPORTANT: A parent or guardian must accompany all patients under 18 to the visit. At the time a minor is treated the parent or legal guardian is responsible for full payment of the service begin provided whether they are accompanied by a responsible party or not. If you are not able to be present for this appointment, you may opt to complete a ‘Consent for treatment on a minor' form. WE STRONGLY RECOMMENDED TO ACCOMPANY THE MINOR AT THEIR FIRST VISIT.
